State Trying To Identify Missing Tax Documents

The state Tax Department of Taxation is alerting Big Island taxpayers about the loss of tax documents filed by the public early this month.

According to the department, a courier bag being sent from the Hawaii District Office and the Oahu District Office was lost on Oct. 4 by the department’s courier service, Security Courier Service.

The bag was discovered missing when the courier arrived at the Hilo International Airport. Police have initiated an investigation into the matter.

The bag was believed to contain approximately 75 tax documents.

They included Business License Forms (Form BB-1) and Cancellation Forms (Forms GEW-TA-RV-1) that were mailed or dropped off at the Kona office between Sept. 24 and Oct. 1.

Also missing are documents that were mailed to the Hilo office on about Oct. 1-3 or dropped off there on about Oct. 3-4. The documents include business license applications, both GET and income tax returns with checks, and payment vouchers.

The department believes it has identified six taxpayers who submitted documents with cash payments, and they will be receiving information from the tax department.

Any taxpayers who believe their documents may have been affected are asked to call the Hawaii District Office at 808-974-6321 for additional information.
