Kealakehe High Sports Physical Clinic

Kealakehe High School, in partnership with the Nakuwauna Foundation, will be holding a Sports Physical Clinic on June 19 from 9:00am-1:00pm in the Kealakehe High gymnasium.  The sports physicals are free to all current and incoming high school students and are valid for one year, allowing students to participate in sports throughout the school year.

Participants will need to have their sports physical forms filled out and signed by a guardian prior to arriving on June 19th.  Forms will need to be completed and in hand for our physicians to complete the physical. 

Forms are available to print on the website (khswaveriders.org) and hard copies are available in the main administration office.   

Students and their families are encouraged to arrive early on June 19th.  Snacks as well as a gift from the Nakuwauna Foundation (while supplies last) will be available.  If you have any questions or know of a qualified person who would like to help, you may reach out to Cassi Vigil-Alip at 808-640-2147.

**Sports physicals do not take the place of the recommended annual well child exam performed by your child’s primary care physician.
